The World today is undergoing a rapid transition and transformation of its demographic pattern. There is an overall increase in life expectancy as we could very effectively conquer many infectious and contagious diseases. Education and cultural development have paved way to high standard of living. Environmental sanitation and better housing, increase in leisure time, technological and scientific advancement, above all, awareness about health and disease have all got a direct impact on our life expectancy. Impressive improvement in maternal and reproductive health, family planning, infant supportive care, immunization programs and scientific approach to child development has led to a decline in the infant mortality with a consequent reduction in family size. These factors led to a new phase of demographic transition model (DTM) both in developed and developing countries. Most developed countries are in Stage IV of DTM while most developing countries are in Stage II or Stage III. Compared to the many developing nations of Asia, India, especially Kerala, is showing a transition in its demographic pattern similar to that of many developed countries.
The demographic transition has got both direct and indirect impact on health care issues and health care planning that demand measures to improve the quality of life from infancy to elderly age. Developing countries, which customarily had youthful populations, now have older and aging ones. This transition has in turn created an emerging set of health problems, that the older population is more vulnerable to chronic diseases, including neurological and psychiatric disorders. Additionally, improvements in health care and the use of life supporting machines are enabling more children with prematurity and developmental disabilities to survive the infancy that lead to the burden of increased developmental disabilities. However we have no effective measures to treat these disabilities or to manage it successfully.
The prevalence and incidence of Developmental disabilities are increasing globally according to epidemiological studies from both developed and developing countries. The World Health Organization (WHO) has estimated that 13% of the world population suffers from one or other form of disabilities. An estimated 1000 million people experience disabilities, more so in poor countries where basic amenities itself is a challenge. The Global Burden of Disease reports that five out of ten most disabling disorders in the world are developmental in nature. Mental Retardation, Autism, Cerebral Palsy, Learning Disability, congenital hearing loss, congenital blindness, genetic and metabolic disorders, infantile stroke, complications of cerebral infection and head trauma are important disorders in this category. A large proportion of the disabled children are in the WHO’s South East Asia region.
According to the WHO, the non-communicable neurological disorders are yet another group of disorders that constitute a major share of Global burden of Diseases. The World Federation of Neurology (WFN) and WHO in their combined study of 1999 revealed that the second most common cause for death is neurological disorders and out of every nine deaths in the world, one is due to a neurological problem. Because of the incomplete and unscientific way of reporting, the prevalence is often documented as erroneously low. Developmental disabilities, Stroke, Dementia, Epilepsy, Parkinson’s disease, Motor Neuron Disease, Post Traumatic and Post infectious Neurological complications are important conditions in this regard.
There are also gaps in the inclusion of developmental disabilities (90% of them are due to neurological causes) and disabilities due to head injury (post traumatic) in the list of non communicable neurological disorders. Therefore ICCONS under SRCCD established since 1998 working in the treatment, rehabilitation, research and development in the field of developmental disabilities and non communicable neurological disorders and other life style diseases proposed to form a registry of patients suffering from these disorders. We propose the compilation of registry for the benefit of people suffering from such problems, developing separate registry for individual disorders and exchange of information on prevention, treatment and rehabilitation of these disorders. Our primary focus is on developmental disabilities and non communicable neurological disorders. We consider life style disorders also as a part of this program since the complication of many of these disorders are neurological in nature and has got importance in the prevention of the risk of developing neurological disorders. It is also important to know the economic burden of these individuals and also educational and career development.
